Please take a moment to
familiarize yourself with our office guidelines.
We have a new patient packet to fill out. You can come 30 minutes early and fill it out in the office or pick it up prior to your appointment.
It is also very important that you bring with you a list of
medications you are currently taking, reports of the most recent X-rays, MRIs,
or cat scans, and notes from prior consultations you may have had with other
physicians concerning the problem you are seeing us for.
Please be aware that your first visit to our clinic is only an evaluation and you will not be getting an injection on this day.
